RN Case Manager - HospicePosition Details- Location: Fort Collins, CO
- Schedule: Monday-Friday, 8:00 AM - 5:00 PM (No nights!)
- Status: Full-time (40 hours/week)
Salary: $85,508 - $93,620 annually
Plus Our Built-In Pay Advantage Program:- Scheduled 72 hours in a 2-week pay period, get paid for 80
- Mileage reimbursement at $0.625/mile (among the highest in the industry)
- Additional earnings through shift differentials, stipends, and on-call pay
Your Role: Independence with SupportAs an RN Case Manager, you will:
- Lead and coordinate care throughout the hospice journey
- Build meaningful connections with patients and families
- Collaborate with physicians, NPs, CNAs, social workers, and clinical leaders
- Provide holistic care-addressing physical, emotional, and spiritual needs
- Work independently in the field while backed by a highly supportive team

What We're Looking For- Graduate of an accredited school of nursing (BSN preferred)
- 1-2 years of experience in hospice, palliative care, home health, med-surg, oncology, public health, or acute care
- Current Colorado RN license and CPR certification
- Valid Colorado driver's license, auto insurance, and ability to travel daily
- Ability to lift up to 35 lbs
