Hospice Registered Nurse (RN) Case Manager • Full-Time | Highland Park, ILJourneyCare Hospice We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Highland Park area.
Schedule & Territory - Territory: Highland Park, IL 60037 and surrounding communities.
- Monday• Friday, 8:30 AM • 5:00 PM. No On-Call.
- One weekend rotation every 12 weeks.
Compensation & Benefits - $80,000 • $97,000 annually (based on experience)

Key Responsibilities - Manage a dedicated hospice caseload as the primary RN Case Manager
- Perform patient assessments, admissions, and individualized care planning
- Deliver skilled nursing care focused on comfort and dignity
- Coordinate care with physicians and interdisciplinary team members
- Conduct routine visits and adjust care plans as needed
- Provide end-of-life support, including family education and pronouncement
- Maintain timely, accurate documentation
- Supervise Home Health Aides in compliance with regulations
Qualifications - Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR and First Aid certification
- Hospice or home health experience preferred
- Strong clinical judgment and communication skills
- Valid driver's license and proof of insurance
