The
RegisteredNurse (RN) Case Manager - Home Care will enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment. The RN Case Manager will provide head to toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and home safety.

Hours: Full-time | Days | Monday - Friday | Holiday rotation (work approximately two holidays per year) | Weekend rotation may apply dependent upon staffing levels.
Territory: This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position of where the patients live would be the southwest section of the Twin Cities Metro area.
A few must-haves for this position:- Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
- Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance are required.
- CPR Certification is required
- Two (2) years of current nursing experience preferred.
- One (1) year case management experience preferred.
- Preferred: Community health and a wide range of Medical-Surgical experience.
Some functions of what you would do as a RN Case Manager:- Perform assessments and procedures, which require knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
- Visit in home clients to evaluate the quality of care being provided and review clinical notes.
- Participate in ongoing interdisciplinary assessment of the patient.
- Provides services that are ordered by the physician as indicated in the plan of care.
- Counsels and educates the patient/client and family in meeting healthcare related needs.
- Completes timely and accurate clinical notes including addressing the patient's/client's progress.
- Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
