Qualifications

• Active RN license in California
• 3+ years of nursing experience
• Previous experience in case management or patient advocacy
• Strong interpersonal and communication skills
• Ability to work independently and manage a caseload effectively

Responsibilities

• Act as a patient advocate to ensure understanding of health status and treatment options
• Assist patients in navigating their treatment plans and resources
• Coordinate care among multiple healthcare providers
• Provide education to patients and families on health management
• Monitor patient progress and adjust care plans as needed
