Qualifications

• Active RN license required
• 5-7 years nursing experience preferred
• Experience in case management essential
• Unit specific experience required
• Strong communication and advocacy skills needed

Responsibilities

• Act as a patient advocate to guide care decisions
• Facilitate understanding of health status and treatment options
• Ensure patients have resources to follow treatment plans
• Coordinate care with healthcare providers and families
• Monitor patient progress and adjust care as necessary
