Qualifications

• Active RN license required
• Previous experience in case management is preferred
• Strong communication and advocacy skills
• Ability to collaborate with multidisciplinary teams
• Knowledge of healthcare regulations and patient rights
• Unit specific experience required

Responsibilities

• Act as a patient advocate throughout treatment
• Educate patients and families on health statuses
• Assist patients in understanding various treatment options
• Coordinate necessary resources for treatment plans
• Evaluate patient progress and adjust plans accordingly
• Liaise between patients, families, and medical teams
