Hospice Registered Nurse Case ManagerSalary: $80-89k/yr BOE
Schedule: Full time | M-F| 8:00AM-5:00PM +(on-call rotation)
Location: Kettering, Ohio, and surrounding areas
Requirements of the RNCM role:- Licensed Registered Nurse
- BLS Certified
- Valid Driver's License and Auto Insurance Coverage
- Reliable transportation
- A minimum of 1 year of Clinical experience
- Hospice experience preferred
About the RNCM role:- Conduct comprehensive patient assessments, including vital signs and medical history review
- Travel to patients' residences to conduct visits
- Develop individualized care plans in collaboration with multidisciplinary teams
- Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
- Perform evaluations for hospice care along with informational visits as needed.
- Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.
- Educate patients and families on treatment plans, medication management, and health maintenance strategies
- Submit complete, accurate, and relevant clinical notes promptly.
- Participate in IDG and case conferences
- Performs on-call duties per policy and as needed
- Conducts an initial and ongoing comprehensive assessment of the patients
- Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.
- Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.
- Provide care plan to Hospice Aide and LPN as part of the IDG team.
- All other duties as assigned
