$500 CONTRACT COMPLETION BONUSOpportunity: RN - Case Management
Job Details:• Work Monday through Friday from 8:00 AM to 5:00 PM with flexible scheduling options (M-F or Tuesday-Saturday)
• Must be available by phone during scheduled shift hours
• Daily huddle at 7:50 AM, which can be attended by phone or Microsoft Teams if traveling to patients
• Open availability to work Saturdays if needed
• No weekend rotation and no holiday expectations
• No on-call responsibilities
• Remote field work serving patients in community settings including private homes, family member homes, assisted living facilities, group homes, and nursing homes
• Locations include Ozaukee and Washington counties
• Manage a caseload of 22-25 patients per week with 6 visits per day
• Serve high acuity patients including those with pleurx drains, PCA pumps, IV antibiotics, TPN, Foleys, wounds, wound VAC, chest tubes, and various drains
• Provide patient and family education, provider communication, and disease management
• Troubleshoot and maintain knowledge of pleurx drains, nephrostomy tubes, trachs, PCAs, and gastric suctioning (training provided on site if needed)
• EMR: EPIC experience preferred
• Laptop, cell phone, and PPE will be provided
• Any scrub color is acceptable
• Must be N95 fit tested prior to arrival
• Required certification: BLS
• Minimum 1 year of home health experience required
• First-time travelers accepted with at least 1 year of home health experience
