Full Job Description
Restorative Nurse (RN/LPN)

Position Type: Full-Time

Pay is based on license and experience.

As a Restorative Nurse, you'll play a key role in enhancing the quality of life for our residents by developing and leading individualized restorative care plans focused on mobility, independence, and dignity.

Key Responsibilities
  • Uphold our commitment to high-quality, person-centered care.
  • Support patients, families, and staff through ongoing communication, education, and recognition.
  • Develop, implement, and evaluate the Restorative Nursing Program.
  • Collaborate with the interdisciplinary team to establish and maintain restorative care standards.
  • Ensure all restorative nursing practices comply with applicable laws and national standards.
  • Provide or supervise direct restorative care services.
  • Perform fall, restraint, and restorative assessments upon admission, quarterly, and as needed.
  • Complete thorough and timely clinical documentation for all restorative care services.
  • Work alongside the Director of Nursing to uphold clinical and regulatory standards of care.
  • Carry out other duties as assigned in support of resident well-being and facility goals.

Qualifications
  • Must be a currently licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.
  • Knowledge of the Minimum Data Set (MDS) process required.
  • Strong interpersonal, communication, and organizational skills.
  • Proficient in electronic documentation and computer use.
  • Ability to work collaboratively and compassionately within a team-oriented environment.
