Starting Pay Ranges:- LPN Base Pay: $40.00 - $45.00 per hour
- RN Base Pay: $46.00 - $50.00 per hour
Shift Differentials (to be added to base rate of pay):- Night Shift: +$2.75
- Weekend Night Shift: +$4.75
Available Schedules:- Night Shift: 7:00pm to 8:00am
All shifts follow a 2-week rotating schedule, workdays and days off vary by week.
We are seeking a dedicated and compassionate
Resident Care Coordinator to join our Assisted Living team. This role plays a key part in ensuring residents receive high-quality, person-centered care while also supporting and guiding our direct care staff. What You'll DoResident Care & Clinical Oversight
- Conduct daily rounds in Assisted Living to monitor resident status and address emerging concerns.
- Lead and complete pre-admission and new resident admissions, ensuring a smooth transition and proper documentation for resident care plans in collaboration with the Nurse Manager (RN) and Assistant Administrator.
- Complete resident annual and change of condition assessments and collaborate with care team to implement resident care plans (NSA/POC).
- Complete timely and accurate incident reports and assist in follow-up investigations.
- Provide oversight and assistance during all residents' dining room mealtimes.
- Complete and update resident elopement profiles.
- Monitor care delivery and complete regular chart and care audits for clinical accuracy and regulatory compliance.
- Primary emergency responder for AL and Independent Residents (tower calls).
Care Coordination & Communication
- Liaise among residents, families, care providers, and other departments to ensure effective and person-centered care.
- Liaise with hospice, home health and home care agencies to facilitate care coordination and to resolve any care issues or concerns.
- Attend care conferences as scheduled.
- Coordinate documentation and other relevant information for resident Long-Term Care Insurance (LTC) claims.
- Attend and contribute to AL stand-up meetings and Sales team collaboration for prospective admissions.
Supervision of Direct Care Staff
- Provide clear direction, coaching, and support to team members to ensure alignment with organizational and departmental goals and achievement of performance objectives.
- Foster a collaborative and inclusive team culture by promoting accountability, professional growth, and open communication.
- Lead by example in problem-solving, decision-making, and conflict resolution, ensuring the team maintains high standards of efficiency and quality.
- Ensure direct care staff have the tools, knowledge, equipment, and identification necessary to support resident care and perform their roles effectively and safely.
- Collaborate with AL leadership on shared supervision of direct care staff.
- Routine competency checks of direct care staff as assigned.
- Assist in orientation of new nurses, Med Techs, and ECAs as assigned.
- Participate in performance coaching, management, and development of ECA team members.
Operational Support
- Collaborate with AL Leadership to identify, document, and implement process improvements aligned with Horizon House's operational efficiency goals.
- Support Infection Control practices and assignments as assigned.
- Provide clinical support for organizational initiatives such as purposeful rounding and other strategic projects.
- Lead emergency operation drills and incidences
- Provide back-up staffing support.

The Ideal Candidate will possess the following qualifications:- Active LPN or RN license in good standing with the State of Washington.
- Two years of experience in nursing (preferred), Assisted Living/skilled nursing/hospital experience, sub-acute/rehab and dementia (preferred).
- One year experience in patient assessment skills, gerontology/geriatric nursing (preferred).
- Current CPR (Certification due within 30 days from date of hire)
- Food Handler's permit (Due within 14 days from date of hire)
- Completion of DSHS Dementia and Mental Health training as required by WA State regulations. (Required to complete these two training sessions within 90 days from your date of hire If you have not completed this requirement previously or cannot locate a copy of your training certifications)
- Microsoft Office Applications (e.g., Outlook, Teams, Word, Excel)
