The salary range for this position is CAD $32.43 - $46.62 / hour
Detailed Overview

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:
The Representative, Disability Management (DM) works with members of the Absence Management & Stay-at-Work team to ensure the integration of services that promote early and safe return to work for employees. The Representative supports the Client Representatives, Disability Management by advising on employee rehabilitation and health-related absences; promoting early return to work (RTW); coordinating early intervention referrals; developing and implementing RTW plans based on medical information and/or functional restrictions; communicating with physicians, rehabilitation and third party providers, managers and unions the specifics of the RTW; monitoring and evaluating RTW programs, maintaining accurate records and utilizing appropriate software to record information and implement effective case management.
Responsibilities

  1. Coordinates the initial Enhanced Disability Management Program (EDMP) process by communicating with key stakeholders including Managers, Unions, rehabilitation providers and third party insurers to facilitate the safe and timely return to work for injured/ill employees.
  2. Assists injured/ill employees in all aspects of their absence including early contact, case triage, initiation of a case management plan, referral/connection to early intervention resources, development and implementation of recovery plans including RTW programs where necessary by utilizing a disability case management model.
  3. Proposes, develops and implements recovery plans with the Client Representatives, Disability Management when the plan involves more in-depth medical case management and/or complex cases and escalates appropriately when necessary. Coordinates the appropriate participants in the development of an individual return to work plan and ensures that the process and outcome are evaluated.
  4. Interprets and applies collective agreement provisions related to EDMP and RTW processes working within the WCB Act and Regulation, Human Rights Legislation and Long Term Disability Benefit Plans; supports and educates Managers, Unions and employees off work on related processes, collective agreements and applicable legislation.
  5. Monitors the status of employees'' participating in EDMP through follow up with Managers, employees and EDMP Union Representatives. Tracks the progress of employees and ensures timely follow up with stakeholders by the accurate maintenance of the WHITE database.
  6. Provides advice and recommendations to employees and managers on the suitability of job duties related to employees current functional status.
  7. Acts as a resource for internal and external inquiries to Workplace Health for information and/or assistance; refers to appropriate Workplace Health team member as necessary.
  8. Performs a variety of clerical duties such as preparing correspondence, reports, minutes, graph utilizing applicable software applications.


Qualifications

Education and Experience

Diploma in Rehabilitation and Disability Management or a related field from a recognized educational institution plus three (3) to five (5) years' recent, related experience in a disability management or rehabilitation setting or an equivalent combination of education and experience.
