Overview

NaphCare is hiring a Release Planner Supervisor to join our team of medical professionals inTucson, Arizona,where we manage all aspects of healthcare for the individuals who are currently incarcerated in thePima County Detention Center. We currently have a Full Time Days position available!

Pay rates range from $37.71-$39.70 per hour based on shift, site needs, hire status, and experience.

As aCorrectional Release Planner Supervisor, you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including performing physical assessments, administering medications, managing chronic illnesses, and responding to emergencies. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs.

Responsibilities

As a Release Planner Supervisor, you will play a critical role in our continued mission to provide quality healthcare that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and not be limited to:

  • Oversees the program
  • Assist patients with a specific plan for medication management that includes involvement of patient discharge planning.
  • Assist patients with the identification of a support network and provide instructions for future care and follow-up.
  • Oversees the coordination of care with community agencies, shelters, employment agencies, and drug and alcohol counselors.
  • Participates in the Collaborative Comprehensive Care Planning meetings, to be convened either in-person or remotely via telephone or video conference on a regular basis at the request of the INVEST Program Manager.
  • Release Planner Supervisor will work closely with Detainee and Crisis Systems Correctional Health Program Manager and team and INVEST Program Manager and team to develop roles and responsibilities for Release Planning team.
  • Release Planner Supervisor must be in attendance at the Medical Administrative Committee (MAC) monthly meeting to provide program updates including lessons learned, service gaps, outcomes, successes and barriers.

Experience / Requirements / Qualifications

  • Bachelors Degree in a human service field bachelor's degree in a related field like social work, or healthcare
  • Must possess an active BLS CPR Certification (American Heart Association required)
  • Knowledge in the development of discharge plans and coordination with community service providers
  • Excellent communication skills are essential for interacting with patients, staff, and physicians
  • Able to work independently and in a team
  • Ability to interact effectively with patients from diverse backgrounds, including those with mental health or substance abuse issues
  • Must have the ability to maintain confidentiality of all proprietary and/or confidential information and integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements
  • Must pass the facilitys criminal background check and drug screening

Physical Capabilities:

Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
