Job Summary:
As a Rehabilitation Liaison, you'll build meaningful relationships with physicians, care teams, patients, and families while guiding individuals through the next step of their recovery journey. Key Position Details:
  • 1.0 FTE (80 hours per two-week pay period)
  • 8-hour day shifts
  • Occasional weekends
  • Required to float between multiple Allina Health locations, including Abbott Northwestern Hospital, Mercy Hospital, Unity Hospital, and other metro-area sites as needed


Job Description:

Principle Responsibilities
  • Coordinates rehabilitation referrals.
    • Establishes and maintains productive referral relationships with key referrer and clinical decision makers.
    • Serves as the primary contact regarding services to the targeted referral sources.
    • Creates and maintains Allina Health rehabilitation presence in the region for all targeted referral sources.
    • Plans communication and public relations-related activities to specific audiences.
    • Sustains Allina Health's position in the rehabilitation market.
    • Educates patients and families at the referral source on the benefits of coming to Allina Health and ensure the referral becomes an admission.
  • Supports the admission process.
    • Determines medical appropriateness of patients, with the decision-making physician.
    • Coordinates the referral of patient.
    • Provides clinical information to inpatient team and working with all parties to ensure appropriate patient information is disseminated.
    • Gathers and transfers patient's medical information to the appropriate inpatient admitting team.
    • Gathers and utilizes patient health information to assist as requested with treatment, payment, and healthcare operations.
    • Educates referral sources regarding changes in regulations, compliance, admissions criteria or outcomes data.
  • Collects data.
    • Prepares regularly scheduled or special reports; collects and analyzes data that is the basis for assigned reports.
    • Monitors and reports on all customer service issues.
    • Coordinates the administrative aspects of a number of referral sources reflecting the referral or admission activities.
    • Maintains a listing of all referral relations activities.
  • Other duties as assigned.


Required Qualifications
  • Bachelor's degree in Nursing, Speech Pathology, Physical Therapy, or Occupational Therapy


Preferred Qualifications
  • 0 to 2 years experience in acute rehabilitation setting


Licenses/Certifications
  • Licensed Registered Nurse - MN Board of Nursing Required if Registered Nurse upon hire or
  • Licensed Speech Language Pathologist - MN Department of Health Required if Speech Language Pathologist upon hire or
  • Licensed Physical Therapist - MN State Board of Physical Therapy Required if Physical Therapist upon hire or
  • Licensed Occupational Therapist - MN Department of Health Required if Occupational Therapist upon hire

Pay Range

Pay Range: $38.52 to $52.80 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
