REHAB DIRECTOR position is now available in our sub-acute/skilled nursing Rehab Department located in
Holden, MA. This position is open to a
PT/ PTA/ SLP-CCC. 3 years experience as a treating clinician is required.
Holden Rehabilitation and Skilled Nursing Center is a 123 bed nursing facility providing long term skilled care and short term rehab services. Holden Rehab is newly renovated and has grown to accommodate many needs with state of the art equipment and mostly private accommodations. Within Holden's new building there are three distinct neighborhoods; short stay, long term and Wachusett Respiratory Care Unit. Each neighborhood has an activity area, family meeting spaces, outdoor courtyards and dining areas with country kitchens. The Town Square is home to our Hair Salon, Consultation Office, and Coffee Shop.

A successful Rehab Director will:
- Ensure efficient and effective operations
- Plan and implement rehabilitation programs and procedures to optimize patient outcomes
- Monitor patient admissions, schedules, MDS assessment periods and data
- Provide administrative rehabilitation supervision
- Serve as a rehabilitation staff mentor
- Provide therapy services as needed
- Be a licensed physical therapist, physical therapist assistant, SLP-CCC or license eligible in the state where employed
- Have experience as a treating therapist in the Skilled Nursing Facility setting
- Candiate must have 3 years experience as a treating clinician.
