Qualifications

• Leadership or management experience is highly desirable.
• Valid driver's license required for travel between locations.

Responsibilities

• Lead and manage a diverse team of healthcare professionals.
• Ensure all practices meet hospice regulations and quality standards.
• Facilitate effective communication among medical and care teams.
• Document patient needs and update care plans during meetings.
• Create and revise patient care policies and emergency guidelines.
• Oversee staff scheduling and encourage a positive workplace atmosphere.
• Act as main contact for communication with families and providers.
• Analyze quality metrics while managing a patient caseload as necessary.
• Be available for after-hours and on-call duties for patient care continuity.
• Optimize resource use and foster collaborative team dynamics.
