Overview The Heater Road Primary Care Clinic provides preventative, acute and chronic care to patients of all ages. Our clinic is fast-paced, with over 50,000 visits annually, including in person and telehealth options!
We utilize multiple different types of care-team members to support our patients' needs, including physicians, APP's, nurses, medical assistants (MAs), licensed nursing assistants (LNAs), diabetic educators, pharmacists, behavioral health clinicians, dietitians, secretaries, and more! The leadership team is very involved in the day-to-day functions, and promote teamwork, collaboration, effective communication, safe and high-quality patient care, and a positive environment for all staff and patients. Together, this team strongly encourages staff to work at the top of their license scope, providing personal connections and promoting continuity of care to patients and their loved ones. The clinic provides comprehensive care to patients during office hours, Monday through Friday, [redacted]. This allows for the team to have consistency with their schedule, promote a positive work/life balance, and maximize their time outside of work.
The role of the clinic nurse is vital to symptom management, triage, patient education, in clinic procedures, as well as collaborative and independent nurse visits. The nursing team is structured to provide support to patients both in clinic and via phone. This position does allow for flexible hours depending on candidate and operational needs. The ideal candidate would be able to provide clinic support with either four 10-hour shifts, or five 8-hour shifts. This position allows for up to one remote day per week.

Responsibilities Practitioner
Utilizes the nursing process to assess, diagnose, identify outcomes, plan, implement and evaluate an individualized plan of care. Utilizes critical thinking and the nursing process to anticipate and recognize changes in patient status, taking action to modify the plan of care or to elevate to the care team as necessary. Practices in accordance with the ANA Code of Ethics to advocate for patients, uphold their autonomy in decision-making, ensure informed consent and assist patients in families in expressing self-determination.
