Where you will workAtrium Health Levine Children's Healthy Futures, located in Kannapolis, NC, offers full-service weight management support for children as young as 2 years old. We help children and their families make healthy lifestyle changes to things like diet and physical activity, in turn lowering their risk of chronic diseases. Our specialized team includes doctors, a nurse practitioner, a registered dietitian, an exercise physiologist and a licensed clinical social worker. Together, we empower your whole family to achieve healthier lifestyles from now to the future.
We're Looking For: - Completion of an accredited or approved program in nursing
- Registered Nurse license issued by the state in which the teammate practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
- One year of Nursing experience needed
What you will do:- Providing education and support to individuals and families.
- Coordinating care and connecting families with community resources.
- Assisting families with accessing healthcare services and follow-up appointments.
- Supporting weight management and wellness plans.
- Providing health education.
- Collaborating with physicians, dietitians, social workers, and other team members.
- Monitoring patient progress and coordinating care.
Demonstrates strong nursing practice through comprehensive patient assessment, care coordination, evidence-based education, and proactive management of patient and family needs. Effectively collaborates with interdisciplinary teams to improve health outcomes, remove barriers to care, and support preventive health initiatives. Consistently promotes patient safety, quality, and engagement while serving as a trusted resource for patients, families, and team members.
