Coverage Area: Coachella Valley

Schedule: Full Time, 4/10s that are within Mon-Fri

Pay: $56-64/hour

Job Summary

Bella Terra is seeking a Registered Nurse who is dedicated to delivering exceptional hospice care. In this role, you will plan, organize, direct, and coordinate home care services while utilizing your nursing expertise and community health experience to support patients and families.

The RN builds upon available community resources to develop and direct services that meet the unique needs of individuals and families within their homes and communities.


Responsibilities
  • Develop and initiate the plan of care and assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s).

  • Initiate proper preventative and rehabilitative nursing procedures and provide services that are ordered by the physician as indicated in the plan of care.

  • Provide patient and caregiver education.

  • Identify discharge planning needs as part of the care plan development and implement plans prior to patient discharge.

  • Ensure that arrangements for equipment and other necessary items and services are available.

  • Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), and/or Home Health Aides (HHAs).

Requirements
  • Active CA RN license

  • Minimum of one (1) year of direct patient care experience; preferably in the hospice setting

  • Possess a current CPR certification

  • Be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
