Qualifications

• Licensed Registered Nurse in Michigan; associate degree preferred.
• Prior experience in hospice, home health, or specific care settings required; one year in Connecticut.
• Willingness to serve on-call and embody the organization's values.
• Valid driver's license and reliable vehicle necessary.
• Physical capability to lift (up to 200 lbs with assistance) and work in diverse environments.
• Ability to adapt to variable conditions like noise and temperature changes.

Responsibilities

• Assess, plan, implement, and evaluate individualized care plans for hospice patients and families.
• Coordinate with physicians and the interdisciplinary team for seamless patient care.
• Maintain accurate documentation of patient records throughout shifts.
• Educate and support patients and caregivers about medication, mobility, and end-of-life preparation.
• Provide emotional and physical support during the death process and assist with 24/7 care coverage.
• Participate in team meetings, supervise hospice aides, and adhere to nursing regulations.
