Qualifications

Responsibilities

• Conduct Start of Care visits to assess clients' needs and devise personalized care plans
• Perform regular reassessments to track patient progress and modify care plans accordingly
• Educate patients and families on managing conditions, adhering to medication, and preventive health strategies
• Collaborate with healthcare professionals, including physicians and therapists, for holistic patient care
• Ensure accurate and compliant clinical documentation per regulations and agency standards
• Supervise Certified Home Health Aides and support care team members
