Position Information

Department: Residential Services

Employment Status: Full-Time Salaried Exempt

Salary:$88,816 Annually

Schedule: Monday-Friday 9:00 AM - 5:00 PM

Work Locations: Agency Residential Programs and Community-Based Sites Throughout Ocean County

What You'll Do
Lead Comprehensive Clinical Care
  • Conduct nursing assessments and monitor the ongoing health status of individuals receiving residential services.
  • Develop, implement, and evaluate individualized healthcare plans that promote long-term wellness and quality of life.
  • Coordinate medical care with physicians, specialists, therapists, pharmacies, and other healthcare providers.
  • Advocate for individuals during medical appointments while ensuring timely follow-up and continuity of care.
  • Monitor health trends, respond to emerging medical concerns, and implement appropriate nursing interventions.
Guide & Develop Clinical Teams
  • Supervise and support Licensed Practical Nurses (LPNs), Medical Case Managers, Certified Nursing Assistants (CNAs), and Certified Home Health Aides (CHHAs).
  • Provide medication administration training, clinical education, and ongoing nursing guidance to residential staff.
  • Serve as a clinical resource for interdisciplinary teams, offering consultation and recommendations that improve health outcomes.
  • Foster a culture of continuous learning, collaboration, and person-centered care.
Ensure Quality & Regulatory Excellence
  • Monitor medical documentation for accuracy, completeness, and compliance with nursing standards and agency policies.
  • Participate in quality improvement initiatives designed to strengthen healthcare services across Residential Programs.
  • Assist with reviewing clinical procedures, nursing protocols, and regulatory requirements to ensure best practices are consistently maintained.
  • Provide clinical consultation to vocational and day program services when healthcare needs arise.
Build Strong Partnerships
  • Collaborate with families, guardians, physicians, hospitals, behavioral health providers, support coordinators, and community partners to coordinate comprehensive care.
  • Maintain clear communication with agency leadership and interdisciplinary teams regarding medical concerns, treatment plans, and ongoing healthcare needs.
  • Participate in audits, licensing reviews, investigations, and regulatory inspections while supporting compliance with DDD, DHS, DCF, and other applicable standards.

Requirements

We're seeking a clinically skilled nursing professional who is passionate about leadership, advocacy, and person-centered healthcare.
  • Graduate of an accredited School of Nursing.
  • Current Registered Nurse (RN) license issued by the State of New Jersey required.
  • Valid New Jersey driver's license in good standing with no more than five (5) motor vehicle points.
  • Minimum of one (1) year of recent medical-surgical nursing experience required.
  • Experience supporting adults with intellectual or developmental disabilities preferred.
  • Strong assessment, communication, organization, leadership, and clinical decision-making skills.
  • Ability to work independently while collaborating effectively with interdisciplinary teams.
  • Must complete all required DDD training requirements and maintain current CPR and First Aid certifications.
  • Ability to successfully complete all required background screenings, including CHRC, CARI, Central Registry, and applicable drug testing requirements.
  • Ability to perform the physical requirements of the position, including lifting, transferring, or assisting with up to 50 pounds when necessary.
