Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team.

Responsibilities

Responsible for maintaining the knowledge and abilities essential to the performance of stated behaviors. Carries out legal responsibilities within the scope of the Nurse Practice Act. Accountable to the NURSE SVC SUPV 2 for overall work performance.

Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Registered Nurse - Pediatric Nephrology works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting for a panel of pediatric, adolescent, and young adult (up to age 21) patients with chronic kidney disease and kidney transplant.

They will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Registered Nurse - Pediatric Nephrology will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely care received in clinic as well as coordination of care. In addition, the Clinical Nurse II will assist in the development of patient care standards and practices.

The Registered Nurse is responsible for:
  • clinic procedures needed by Renal patients including but not limited to central line care and dressing changes, lab draws, port access, medication administration.
  • coordinating outpatient services and needs including assisting with insurance authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care.


Qualifications

Required Qualifications:
  • Minimum of one year full-time RN experience in pediatric inpatient setting
  • Recent RN experience in pediatric outpatient specialty care coordination
  • Recent experience with pediatric patient population in inpatient and/or outpatient setting
  • Knowledge and ability essential to the successful performance of the duties assigned to the position.
  • Recent experience in the use of electronic medical record documentation
  • Experience with use and care of portocaths, central lines and drawing labs required.
  • Phlebotomy experience required
  • This position requires flexibility to orient and work at all UCSF Medical Center locations.


Required License/Certifications:
  • Current licensure as a Registered Nurse as defined by the California Board of Registered Nursing.
  • Current American Heart Association CPR/BLS certification


Preferred Qualifications:
  • Bachelor's of Science degree in Nursing (BSN)
