Role: Registered Nurse
FTE: Permanent, 30 - 37.5 hours (1.0 FTE)
Location: 421 Greenbrook Drive, Kitchener, Ontario
Position Summary
As a Registered Nurse (RN) with New Vision Family Health Team, you will provide comprehensive primary care nursing services to support the health and well-being of patients across the lifespan. Working both independently and collaboratively with physicians, nurses, allied health professionals, and administrative staff, you will contribute to the delivery of effective, timely, and patient-centred care while supporting patients in achieving their health goals. The RN will also play an integral role in supporting the team's Diabetes and Hypertension Program, as well as other programs, services, and initiatives offered by the Family Health Team, helping to promote chronic disease management, prevention, health education, and overall patient wellness.
Key Responsibilities
Patient Care & Treatment
- Conduct comprehensive patient assessments, including vital signs monitoring, physical assessments, screening, and data collection.
- Implement medical directives and perform clinical procedures within RN scope of practice.
- Administer medications and injections in accordance with College of Nurses of Ontario (CNO) standards, clinic policies, and medical directives.
- Provide patient education, monitoring, follow-up care, communication of test results, coordination of follow-up, and responses to patient inquiries within RN scope of practice.
- Participate in chronic disease management programs and preventative health initiatives.
- Maintain accurate, concise, and confidential documentation in the electronic medical record (EMR) and preventative care records.
Team & Clinical Support
- Collaborate closely with physicians and the interdisciplinary team to provide integrated nursing support and deliver coordinated, patient-centred care.
- Support physicians by anticipating clinical needs, assisting with care planning, and facilitating timely follow-up.
- Participate in quality improvement initiatives and contribute to ongoing program development and evaluation.
- Uphold the Standards of Practice of the College of Nurses of Ontario and all applicable clinic policies and procedures.
Qualifications
- Bachelor of Science in Nursing (BScN) from an accredited nursing program or recognized equivalent.
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Current membership with the Registered Nurses' Association of Ontario (RNAO).
- Proof of professional liability insurance.
- Current Basic Life Support (BLS) / CPR certification.
- Demonstrated ability to work independently and collaboratively within an interdisciplinary primary care team.
- Certified Diabetes Educator (CDE) certification is an asset; willingness to obtain certification is also considered an asset.
- Strong organizational skills with the ability to prioritize, manage time effectively, and adapt to a fast-paced clinical environment.
- Excellent communication and interpersonal skills.
- Demonstrated sensitivity to social, cultural, and individual patient needs.
- Successful completion of a Criminal Record Check with Vulnerable Sector Screening.
- Completion of health screening requirements (MMR, Tdap, Varicella, and Hepatitis B).
Compensation & Benefits
- Hourly wage ($42.40 - $44.60), commensurate with experience (ie: Certified Diabetes Educator)
