Job Summary:  The Registered Nurse (RN) provides holistic patient care as a key member of the Mobile Health team. The RN performs essential clinical tasks in a mobile healthcare setting, ensuring efficient, high-quality patient care across diverse and often underserved communities. This position combines clinical expertise with adaptability, delivering care in non-traditional environments including mobile clinics, shelters, agricultural sites, correctional settings, and community-based outreach events. The RN provides care across multiple Mobile Health service lines, including Mobile Primary Care, the Center for Addictions Triage and Treatment (CATT), Low Acuity Transitional Services (LATS)/Medical Respite, the Farmworker Outreach Program (FOP), Correctional health services, and community-based outreach programs.

Essential Duties and Responsibilities:    

•    Conduct comprehensive assessments of patients, including medical history, current health  
•     Conduct comprehensive patient assessments, including medical history, current health concerns, and vital signs in mobile and field-based settings 
•    Identify patients with significant health risks, including those related to chronic conditions, substance use, and social determinants of health 
•    Provide patient education on diagnosis, treatment plans, medications, infection control, and preventative care tailored to community needs 
•    Support chronic disease management (e.g., diabetes, hypertension, asthma), including lifestyle education and medication adherence 
•    Triage walk-in patients and coordinate care in collaboration with providers 
•    Administer medications (oral, IM, IV) under provider supervision as appropriate 
•    Perform point-of-care testing and laboratory procedures 
•    Manage high-risk patients and coordinate follow-up care across multiple service locations 
•    Maintain infection control standards within mobile and field environments 
•    Utilize telehealth tools and remote consultation as needed 
•    Coordinate patient flow and assist with mobile clinic setup and breakdown 
•    Provide group and community-based health education
•    Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
•    Perform other duties as assigned.
•    Valid driver’s license, reliable transportation, safe driving record and insurance coverage required.

Clinical Role & Operations
•    Lead and support clinic flow across mobile sites, ensuring efficient and safe patient care delivery 
•    Delegate tasks appropriately to LPNs, MAs, and support staff in alignment with scope of practice 
•    Coordinate care across multiple service lines and partner organizations 
•    Adapt clinical workflows based on site type (respite, outreach, correctional, event-based care) 
•    Support operational readiness, including supplies, equipment, and site setup

HIPAA Requirements:
The Registered Nurse – Mobile Health has access to PHI to create and maintain an accurate and up-to-date health record. Applying the minimum necessary standard of HIPAA, the designated record sets to which this employee will have access include all sections of the dental and medical record, patient demographic information in the practice management system, incoming records, reports, results, consultations, etc. The [position title] should read the content of these records only to the extent needed to accomplish the assigned task (e.g. filing or disclosure).

Knowledge, Skills and Abilities Required:

•    Strong commitment to trauma-informed care, cultural humility, harm reduction, and health equity 
•    Ability to provide compassionate, high-quality care to vulnerable and underserved populations 
•    Experience working with patients experiencing homelessness, substance use disorders, and complex medical and social needs 
•    Strong wound care, vaccine administration, infection control, and acute care skills 
•    Ability to work effectively in non-traditional, high-volume, and mobile environments 
•    Strong coordination and communication skills across multidisciplinary teams and partner organizations 
•    Proficiency in medical documentation and electronic health records (Epic preferred) 
•    Bilingual English/Spanish preferred

Education and Experience Required:    
•    Current Oregon RN License.
•    Experience in community health, mobile health, or working with underserved populations preferred.

•    Standing: 15%

•    Lifting/Carrying: 15% (Occasional lifting of equipment up to 30 lbs.)
•    The RN must be able to assist in setting up and dismantling mobile clinics, including moderate physical labor.
Working Environment/Physical Hazards:
•    Work in a variety of environments, including outdoor settings, community centers, and mobile units.
•    Potential exposure to bloodborne pathogens and hazardous materials.
•    Adherence to strict infection control measures within mobile units.
•    Comfortable working in fast-paced, dynamic environments
•    The ideal RN will be comfortable utilizing their full scope of practice including assessment, planning, implementation, supervision and delegation of appropriate tasks to licensed and unlicensed team members (LPN's, CNA's, and ancillary staff) as outlined by the Oregon Nurse Practice Act
Equipment Used:
•    Medical equipment (e.g., autoclave, pulse oximeter, glucometer) and mobile-specific equipment as required for remote care.
•    Office equipment (e.g., laptops, mobile devices) for documentation and communication.

Immunization:
Staff members must meet immunization requirements as stated in VGMHC’s immunization policy and state and federal guidelines.
