- Serve as a Charge Nurse, Staff Nurse, or Assessment Coordinator within a long-term care setting dedicated to Veterans' healthcare services.
- Provide direct nursing care to Veteran residents with chronic medical conditions, physical disabilities, cognitive impairments, behavioral health needs, and age-related health concerns.
- Conduct comprehensive nursing assessments, including full physical assessments, cognitive and mental status evaluations, and ongoing condition monitoring.
- Develop, implement, and evaluate individualized nursing care plans in collaboration with interdisciplinary care teams focused on Veteran-centered care.
- Coordinate care with physicians, specialists, therapists, social workers, Veterans, and family members or designated support persons.
- Supervise, guide, and support paraprofessional and non-professional nursing staff to ensure high-quality resident care.
- Direct and coordinate nursing operations during assigned shifts, ensuring appropriate staffing coverage, supplies, and clinical readiness.
- Maintain accurate, timely, and compliant documentation within electronic medical record (EMR) systems.
- Monitor resident conditions, administer medications, and perform treatments in accordance with nursing standards, VA/state regulations, and facility protocols.
- Participate in infection control programs, quality improvement initiatives, risk management activities, and Veteran care enhancement programs.
- Ensure compliance with federal/state regulations, Veterans' care standards, facility policies, and professional nursing practice guidelines.
- Respond to medical emergencies and provide appropriate life-saving interventions, including CPR, AED use, and emergency stabilization procedures.
- Perform a wide range of clinical procedures, including IV therapy, wound care, catheter care, medication administration, specimen collection, respiratory treatments, enteral feeding support, and ventilator care as applicable.
- Participate in interdisciplinary care conferences, Veteran care plan reviews, discharge planning (when applicable), and ongoing care coordination activities.
Job Requirements: Job Requirements: - Current Registered Nurse (RN) license issued by the Pennsylvania State Board of Nursing, or a valid non-renewable temporary practice permit.
- 2+ years of experience preferred
- Experience providing nursing care in long-term care, residential, behavioral health, developmental disability, rehabilitation, or similar healthcare settings preferred.
