Full Job Description
  • Primary Location: 385 Southbridge St, Auburn
  • Department: Primary Care
  • Schedule: Full time, 38.75 hours/weekly, Monday through Friday, scheduling between 8:30AM - 5:00PM

Overseeing assigned employees and activities for the efficient administration of nursing activities. Coordinates daily clinical operation needs in assigned areas. Provides direct patient care utilizing the nursing process and adhering to standards of nursing practice, department-specific policies, procedures, and protocols.

Primary Responsibilities:

  • Providing leadership to the team and monitoring to ensure daily work, including but not limited to inbasket work, telephone triage, direct patient care, and floor work, is being done in a timely manner
  • Completes daily staffing and provider assignments. Assessing staffing needs and adjusting coverage as needed throughout the day
  • Acting as an additional resource during the work day by conducting daily rounds to answer any questions or concerns and reinforce Reliant policy, procedures, and workflows
  • Lead will inform Nursing Supervisor of needed staff re-education
  • Initiate daily nursing communication via jabber to help facilitate open communication in the department
  • Monitor in basket work to ensure appropriate patient response times
  • Assist Nursing Supervisor with Practice, Health, and Safety (PHS) audits of your designated pods
  • Monitor logs weekly, ensuring they have been checked at required frequency and excursions and/or errors have been appropriately documented and resolved. Logs include, but are not limited to, medication refrigerator and freezer, chain of custody, accession, etc.
  • Provide feedback to Nursing Supervisor for staff reviews
  • Assist will all medical emergencies within the building. Identifies emergency situations and initiates appropriate nursing orders/interventions. Meets patient needs promptly
  • Provides effective patient care directly to patients or through telephone management
  • Administers, monitors, and evaluates all therapeutic interventions, including but not limited to safe medication administration according to Standard Policies and Procedures and applicable state and federal laws, including but not limited to immunizations and department-specific medications
  • Administers intravenous medications, including limited IV push medications depending on patient population, e.g., Urgent Care versus Primary Care patients, as required
  • Consistently documents all the aspects of patient care and patient education. Reports pertinent information to physician
  • Ensures nursing judgments and plan are based on physical, psychosocial, cultural, and diverse needs
  • Delegates patient care responsibilities to other members of the healthcare team, when appropriate
  • Utilizes patient care equipment appropriately and safely. Monitors and reports any needed repairs and/or maintenance of clinical equipment
  • Completes all organizational and department-specific nursing/clinical and educational competencies
  • Complies with health and safety requirements and with regulatory agencies such as DPH, etc.
  • Enhances professional growth and development through educational programs, seminars, etc.
  • Attends a variety of meetings, conferences, and seminars as required or directed
  • Regular, reliable, and predictable attendance is required

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $40.00 to $55.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
