Join PACE CNY as a Regustered Nurse, Intake!

 

The Registered Nurse, Intake assists with the marketing of PACE CNY to the referral community, follows up on all referrals to the program, coordinates the intake process for potential new enrollees, strives to meet the enrollment goals of PACE CNY, and assists in orienting all PACE CNY employees and contracted providers about PACE CNY and prohibited marketing practices.

Schedule: Monday-Friday: 08:00 AM- 04:30 PM

Salary: $80,101 -$89,960 Annually

No Holidays or Weekends!

Responsibilities

Job Duties Specific to This Position:

  • Assists in marketing PACE CNY services by meeting with community groups, hospital discharge planners, home care agencies, and other potential referral sources on a regular basis.
  • Returns all phone calls within one working day.
  • Meets with potential participants and their family members to discuss the philosophy of PACE CNY, enrollment criteria, managed care, capitation, and Medicaid eligibility/payment.
  • Provides in-services and presentations to Loretto employees and potential referral sources regarding PACE CNY care options and services.
  • When possible and appropriate, obtains information that facilitates the intake process (financial statements, medical releases, Medicaid and housing applications, etc.) from other providers and family members.
  • Maintains regular contact with individuals referred to PACE CNY.
  • Follows up on the intake/enrollment process to ensure smooth transition; identifies areas for improvement and take appropriate corrective actions.
  • Provides effective tours to potential participants and families at PACE CNY sites.
  • Coordinates intake evaluations of potential participants by providing written information to the PACE CNY Team, Loretto Finance, and the Onondaga County Social Services Resource Center. Provides ongoing communication with families and/or referral sources during the intake process.
  • Maintains adequate documentation regarding the status of the intake process.
  • As needed, attends scheduled intake and assessment (I&A) meetings to provide interdisciplinary team with potential participant information that will help determine whether or not a potential participant should be enrolled in the program.
  • Maintains statistical data regarding marketing outreach, intakes, referral sources and other PACE specific data. Prepares and submits accurate statistical reports including referrals/enrollments and marketing outreach log monthly.
  • Reports the status of referrals and intakes to the Director of Marketing & Intake weekly.
  • Assists in the development of marketing materials and opportunities to attract additional participants.
  • Maintains current knowledge of Medicaid and Medicare A, B and D programs guidelines.
  • Follows DOH and CMS Marketing Guidelines and adheres to the Prohibited Marketing practices.
  • Maintains adequate knowledge of other community resources available for services.
  • Maintains up-to-date information on changing financial options and rules.
  • Maintains current knowledge of third party regulations and follows all relevant guidelines.
  • Networks with other community providers to obtain necessary and current information.
  • Participates in meetings, conferences, and seminars as required.
  • Establishes and maintains an open, productive, and cooperative working relationship with other staff, outside agencies and provider hospitals.
  • Maintains appropriate relationships with other professional and human service groups in the community and serve on community committees when appropriate.
  • Coordinates intake evaluations for potential participants with other Loretto programs and third party regulatory agencies such as DSS, as required.
  • In conjunction with the appropriate management staff, ensures the program adheres to applicable state, federal and accrediting body regulations.
  • Participates in quality assurance initiatives as appropriate or as requested by Director of Marketing and Intake.
  • Completes all annual corporate and PACE CNY re-orientations in a timely manner.
  • Participants in and conducts in-services. Attends seminars, intra-agency sessions, and conferences to maintain state-of-the-art practices and techniques.
  • Is responsible for promoting, role modeling and fostering compliance in the workplace. Communicates expectations to employees regarding compliance oriented behavior. Demonstrates adherence to all compliance policies and procedures and the code of conduct.
  • Ensures the highest quality of care by performing responsibilities according to the highest professional standards.
  • Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

Qualifications
  • Degree from an accredited school of nursing. BSN or BS in related field preferred.
  • Current NYS license and registration as an RN in good standing required.
  • Some background in marketing communications or sales preferred.
  • Experience in the delivery and marketing of elder services preferred.
  • Must possess a valid driver27s license without restrictions and a reliable, insured vehicle.
  • Current knowledge of best practice nursing standards and care.
  • Proven patient assessment skills.
  • Self-directed and able to maintain schedule.
  • Ability to exercise good judgment and maintain confidentiality.
  • Proven organizational, time management, and multi-tasking skills
  • Strong verbal and written communication skills.
  • Strong customer service and interpersonal skills.
  • Basic computer skills in MS Office preferred.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
