Qualifications

• Active Registered Nurse or Licensed Practical Nurse license in New York.
• At least 2 years of RN experience in home healthcare or medical/surgical settings, such as critical care or ER.
• Proficient in assessing and responding to patient and family needs at home.
• Professional liability insurance as per VNSW requirements.
• Competent in using industry technology, like tablets, for patient care.
• Access to a reliable vehicle for field work.

Responsibilities

• Assess patients' needs and develop personalized care plans.
• Implement and evaluate the effectiveness of patient care interventions.
• Coordinate care with patients, families, and healthcare team members.
• Provide supervision and training to Home Health Aides in patient care delivery.
