Job DescriptionThe Heart Function Clinic Registered Nurse is responsible for performing assessments, developing a plan of care and implementing and evaluating the care of patients with Heart Failure in coordination with the Cardiologist and Pharmacist.
- Performing clinical assessments of the new and returning patients to the clinic
- Utilize the physical assessment and critical thinking in assessing patient symptoms or lifestyle changes in conjunction with heart failure guidelines and protocol to determine appropriate patient care plan
- Coordinate the plan for patient care between the Cardiologist and Pharmacist assessments
- Provide education to the patients regarding lifestyle and dietary adjustments associated with the heart failure conditions
- Work collaboratively with the patient and the patient family to promote the appropriate condition management
- Document the assessment information in the clinic specific information management systems according to the given guidelines for patient information management

- Current registration in good standing with the College of Nurses of Ontario
- Current BCLS-HCP certification required at time of hire
- Current ACLS certification required at time of hire
- Demonstrated clinical expertise in care of patients with Cardiovascular disease is required
- Demonstrated knowledge of Heart Failure is required
- Demonstrated knowledge of ECG interpretation is an asset
- Demonstrated initiative and organizational skills
- Demonstrated teaching skills
- Experienced in verbal and communication skills
- Satisfactory attendance and performance record
- Physically able to perform the job
Additional InformationHours: Monday - Friday, 7.5 hour day shifts (hours are subject to change based on operational needs)
Salary: $41.15 to $58.98 per hour
