Summary of responsibilities

Reporting to the Manager, System Navigation, Geriatric Mental Health and Community Withdrawal Management, the Registered Nurse will provide support and consultation for clients with dementia or delirium, who are exhibiting responsive behaviors. Working with the client, their family/caregivers, as well as the interdisciplinary team, the nurse will offer case management, collaborative care planning and consultation to support complex transition across the continuum of care from the community to Retirement Home and assisting with the Long Term Care Transition as needed. Services will be offered in the community/home environments. Through consultation, formal and informal training, skill development and knowledge transfer, the nurse will work with the Geriatric Day Hospital, Geriatric Mental Health Services, hospital staff and community partners, to build capacity in managing clients with dementia and responsive behaviors as well as embody a culture of caregiver inclusion in the patient's plan of care.

Requirements of the job
• Holds current Certificate of Competence from the College of Nurses of Ontario
• Holds current certification in Basic Cardiac Life Support
• Demonstrated thorough knowledge of community resources and support services that facilitate safe effective discharges and allow patients to remain in the community
• Minimum 3 years of recent clinical experience with Medical/Surgical geriatric patients either in hospital setting or in the community and/or minimum 2 years recent experience working with psychogeriatric patients in an adult psychiatric facility or community mental health clinic
• Demonstrated strong knowledge of the Health Care Consent Act
• PIECES and Gentle Persuasive Approach Certification
• Demonstrated working knowledge of pharmacology and other therapeutic modalities, as well as the Mental Health Act
• Demonstrated experience in a liaison role with external partners, such as LHIN, primary care practitioners, LTC homes, retirement homes, community and support services agencies
• Demonstrated initiatives to enhance nursing knowledge and professional competence through attendance and participation at conferences, workshops, continuing education programs, and/or other related activities
• Good computer skills and good working knowledge of Microsoft Office,
PowerPoint, email, internet, etc.
• Demonstrated regular, punctual attendance
• Demonstrated commitment to patient safety
• Valid driver's license and access to a vehicle
• Ability to meet the physical demands of the position
• Functional level in English, oral and written

Assets:
• Demonstrated functional abilities with Cerner electronic health record and CRMS
• CNA Certification in Geriatrics or Mental Health Nursing
• Geriatric Assessor Training through the Regional Geriatric Program, or equivalent
• Teepa Snow Coaching Certification
• Experience in Community Nursing, Case Management, or Management and Development of Patient Care Plans
• Knowledge of Long-Term Care Regulations
• Functional level in oral French (with testing completed)
