The RN Home Services Case Manager leads patient‑centered care in the home by developing individualized plans that promote independence, safety, and quality of life. This role serves as the primary nurse from admission through discharge, completes OASIS assessments, and coordinates care across caregivers, providers, and community resources to ensure seamless, high‑quality outcomes.

Minimum Qualifications:- Current Registered Nurse (RN) license in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
- Bachelor of Science in nursing (BSN) from an accredited institution (degree verification required).
- RNs must obtain their BSNs within four (4) years of hire or promotion date. If there is an existing education agreement, that agreement will take precedence.*
- Basic Life Support Certification (BLS) for Healthcare Providers.
- Current driver’s license, insurance, reliable transportation, and an acceptable driving record.
- Unit specific certification as required (i.e., ACLS, PALS, etc.).
- Demonstrated clinical nursing experience.
- Demonstrated knowledge of home health or hospice.
- Ability to communicate effectively both verbally and in writing.
Note: BSN requirement effective as of the last job description update on 4/27/2025. Employees hired or promoted before this date are held to the requirements in place at the time of hire or promotion.
Preferred Qualifications:- Case Management Certification
- One (1) year experience working in home health or hospice.
- Three (3) years of clinical nursing experience.
- Experience working with third-party payors
- Previous experience in case management, utilization review, and/or discharge planning.

Physical Requirements
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
- Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
- Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
- Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
- May be expected to stand in a stationary position for an extended period of time.
- For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
