Qualifications

• Active Maryland Registered Nurse (RN) license and insured.
• Strong collaboration skills to work within a multidisciplinary team.
• Excellent organizational, communication, and clinical documentation skills.
• Current CPR certification required.
• Privileged to perform all client treatment coordination duties.
• Clean driving record with a valid driver’s license.
• Demonstrated ability to effectively engage with clients in home environments.

Responsibilities

• Connect clients to necessary primary and behavioral health care.
• Assist clients in understanding discharge instructions and following doctors' orders.
• Develop acute care plans incorporating client input and approval.
• Create preventive care plans after the first thirty days with client involvement.
• Conduct assessments and provide outreach services in client homes.
• Document care plans and outcomes as directed using electronic medical records.
• Motivate clients to engage in self-care and manage chronic conditions effectively.
