Carewrap Registered Nurse (RN)

Full-Time Professional
Location: Salisbury, MD and surrounding counties
Salary Range: $80,000 - $95,000 annually
Schedule: 4-day, 36 hour work week (Monday-Thursday)

Position Summary

The Carewrap Registered Nurse position carries out services designed to assist recuperation, therapy, and healing at home and prevent hospital use.

Key Responsibilities

  • Connect clients to primary and behavioral health care as indicated.
  • Assist clients to understand discharge instructions, fill prescriptions, and follow doctors' orders.
  • Develop immediate/acute care plans with clients' input and approval.
  • Develop preventive care plans after the first thirty days with clients' input and approval.
  • Carry out assessment, care, and outreach to homes, provide transportation as requested in all parts of catchment area (four counties).
  • Document activities, care plans, and outcomes as directed.
  • Motivate clients to take an active and effective role in self-care and lifestyle best suited to coping with any chronic conditions.
  • Using electronic medical records, input data and following through seeing services to their completion.
  • Maintain total confidentiality and adhere to HIPAA law, CARF accreditation standards and Maryland State Regulations.
  • Other duties as assigned by the Director of Nursing.
