Key Responsibilities:- Deliver direct nursing care and monitor resident health
- Develop and implement individualized care plans
- Maintain accurate documentation using EMR systems
- Collaborate with healthcare professionals and families

Qualifications:- Valid RN license in Pennsylvania
- Strong clinical and communication skills
- Proficiency in electronic medical records
- Detail-oriented, adaptable, and team-focused
- Completion of Fellowship Orientation and pre-hire assessment
