Responsibilities

Job Description:

Purpose of Job:

Improve the overall health of the communities we serve by providing individualized nutritional support to patients for their particular situation as follows:

Essential Duties and Responsibilities:
  • Meets directly with patient to identify dietary needs through nutritional assessment based on nutrition-focused physical findings and patient history.

  • Provides nutritional counseling either individually or in a group setting; identifying goals and supporting education with evidence-based care.

  • Develops detailed care plans and documents as part of the electronic health record in accordance with established program standards.

  • Assesses, plans and implements the delivery of medical nutrition therapies in collaboration with the patients inter-disciplinary team.

  • Monitors and evaluates patient's progress through routine nutrition interventions to support positive health outcomes.

  • Participates and supports patients in various program committees; including developing educational programs for various ethnic, cultural and social groups to teach nutrition education.

  • Remains up to date on evidence-based research and recommends ways to expand and improve nutrition programs.

  • Participates in quality improvement of the program in conjunction with other services CHAS Health offers.

  • Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

Qualifications:

Education/Experience: Bachelors degree in Nutrition or related field required. Registered Dietitian credentialed with the Commission on Dietetic Registration and eligible for state licensure. Experience in chronic disease including HIV and Diabetes and/or maternity nutrition counseling preferred.

Skills: Excellent oral and written communications skills required. Computer skills required. BLS (CPR/AED)

Physical Demands:Dietitian must be able to move around the facility between 1/3rd and 2/3rds of the day, as well as sit at a workstation or in patient rooms. Using hands occurs over 2/3rds of the day, while reaching with arms occurs less than 1/3rd of the day. Communicating is also required over 2/3rds of the day, while climbing, stooping or crawling is minimal. Lifting up to 10 lbs occurs about half the time, while up to and above 25 lbs. occurs only occasionally.
