Regional RN -Crown Park, Cortland Park, Central Park and Colonial Park

Travel reimbursement

As a Regional RN Clinical Consultant, you will provide clinical support and oversight to skilled nursing facilities, partnering with facility leadership teams to promote quality care, regulatory compliance, and positive resident outcomes. Your responsibilities will include:
  • Participating in mock surveys, focused clinical reviews, and follow-up audits
  • Supporting survey readiness and regulatory compliance
  • Assisting facilities with Plans of Correction and QAPI action plans
  • Reviewing clinical systems, including falls, wounds, infection control, medication management, care planning, behavior management, and resident safety
  • Providing coaching and education to DONs, nurse managers, licensed nurses, and frontline staff
  • Identifying root causes of clinical concerns and helping facilities implement sustainable corrective actions
  • Monitoring trends in quality measures, rehospitalizations, incidents, grievances, and survey findings

Requirements:
  • Current Registered Nurse license in good standing
  • Prior Director of Nursing experience in a skilled nursing facility or long-term care setting is required
  • Strong knowledge of CMS Requirements of Participation and state survey process
  • Demonstrated experience with survey readiness, Plans of Correction, QAPI, and clinical systems oversight

  • Ability to travel to assigned skilled nursing facilities as needed
  • Proficiency with electronic medical records and clinical documentation review

Preferred Qualifications:
  • Multi-facility clinical consulting or regional nurse experience
  • Experience with Immediate Jeopardy removal, focused infection control reviews, quality measure improvement, and mock survey processes
  • Strong background in DON mentorship and nursing leadership development
