Overview

We are seeking a strategic and collaborative leader to serve as Director of Care Management, overseeing hospital and ambulatory care management teams and programs. This role provides operational oversight, strategic direction, and leadership to ensure coordinated, patient-centered support across the continuum of care. Responsibilities include improving patient outcomes, reducing unnecessary utilization, optimizing healthcare resources, and partnering with clinical, operational, and external stakeholders to drive high-quality care delivery. The Director is responsible for resource allocation, removing operational barriers, and influencing key initiatives across multiple stakeholder groups. This individual will serve as the market's subject matter expert within their area of responsibility and play a critical role in advancing care management strategies and organizational goals.

Responsibilities

  • Provide strategic leadership and operational oversight for Integrated Care Management (ICM) programs across hospital and ambulatory settings.
  • Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers to deliver high-quality, patient-centered care.
  • Build strong partnerships with clinical leaders, community organizations, and cross-functional stakeholders to support seamless care integration.
  • Drive process improvement initiatives focused on reducing readmissions, improving patient flow, optimizing length of stay, and enhancing care coordination outcomes.
  • Ensure compliance with CMS, DNV, OSHA, OIG, and other federal and state regulatory requirements.
  • Coach, mentor, and develop team members while fostering a culture of collaboration, accountability, and service excellence.
  • Analyze clinical, operational, and financial performance metrics to support strategic growth, quality improvement, and organizational goals.


Qualifications

Education:
  • Bachelors of Science in Nursing (BSN), Bachelors of Social Work, (BSW) or equivalent education in health care management Required

License(s)/Certification(s):
  • Current licensure in good standing to practice as a Registered Nurse or Licensed Social Worker

Experience:
  • 3-5 years of progressive leadership experience in Care Management or Population Health or Healthcare Management required
