Job Details
Interprets physicians' prescriptions and dispenses medications for high dollar specialty and other medications within regulatory, accreditation, and policy guidelines. Provides medication therapy management by actively managing drug therapy; identifies, prevents and resolves medication-related problems. (e.g. Counsels patients and/or medical staff and handles problem resolution). Completes patient records, initial assessments, medication profiles, initial fill notes and continuing progress notes. Participates in the coordination of care to meet patient outcomes and goals including, initial referral, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services.

Interacts with internal and external clinical and non-clinical personnel to support benefits investigation, financial assistance and prior authorization needs. Supports activities for mitigating patient side effects and promoting therapy adherence. Engages in practice pull through for specialty and other orders. Aids in development and maintenance of patient management program. Participates and undertakes initiatives and projects impacting critical clinical, operational and financial areas.

OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions:
  • Documents any and all interactions with patients and prescribers in the appropriate systems in
  • compliance with protocol.
  • Performs On-Call activities as required.
  • Adheres to all pharmacy, state and federal laws and regulations.
  • Performs other duties as assigned.

POSITIONS, SUPERVISED OR DIRECTED (TITLES AND APPROXIMATE NUMBERS):
  • Specialty Pharmacy Representatives I and II, Rotation IPPE and APPE students


EDUCATIONAL/TRAINING REQUIREMENTS:
  • PharmD degree or Bachelor of Science Degree in Pharmacy with at least 3 years of Pharmacy experience or 1-2 years of Specialty Pharmacy experience is preferred.


CERTIFICATES, LICENSES, AND REGISTRATION:
  • Valid Pharmacist licensure in the State of Pennsylvania


EXPERIENCE REQUIREMENTS:
  • Prior specialty pharmacy experience or residency preferred


ADDITIONAL INFORMATION:
  • Ability to communicate clearly and effectively, verbally and in writing, with physicians/medical staff, patients and insurance companies.
  • Strong organization skills, with ability to multitask, collate data from many sources and manage several tasks at any one time.
  • Excellent problem-solving skills and the ability to adapt to the changing needs of the organization.


Additional Preferred Qualifications:
  • Extensive working knowledge of URAC and The Joint Commission accreditation standards who has successfully lead accreditation processes; Quality, Safety and Auditing initiatives.
