Description

We're seeking a detail-oriented, clinically driven PV1 Pharmacist to join our Clinical Innovation Center team. In this role, you'll serve as the first pharmacist review in the prescription verification process, helping ensure medication orders are accurate, safe, and appropriate before they move forward in the fulfillment workflow.

Location: Remote or Hybrid for candidates local to Pheonix, AZ

Salary : $125k-$130k (dependent on experience)

Schedule: Monday - Friday 1:00 pm - 9:30 pm and rotating weekends

As a PV1 Pharmacist, your decisions directly influence patient safety, medication accuracy, and quality of care. By identifying potential issues before medications are dispensed, you'll help prevent medication errors, improve clinical outcomes, and provide peace of mind to patients, providers, and care teams across long-term care communities.

Your work will support a seamless pharmacy operation while ensuring every order meets the highest standards of accuracy and clinical excellence.

Key Responsibilities:

  • Perform first verification (PV1) review of prescription orders before they advance to production and fulfillment.
  • Conduct comprehensive clinical reviews to evaluate medication appropriateness based on patient age, diagnosis, laboratory values, allergies, drug interactions, and duplicate therapies.
  • Verify prescription and patient information entered by pharmacy technicians or received through electronic prescribing systems.
  • Identify and resolve discrepancies, incomplete orders, and potential medication-related concerns.
  • Communicate directly with prescribers and healthcare providers to clarify orders, dosing, therapies, or documentation requirements.
  • Ensure compliance with federal and state pharmacy regulations, company policies, and professional standards.
  • Accurately document interventions, provider communications, and order resolutions.
  • Collaborate with pharmacists, technicians, nurses, and facility staff to support timely medication processing.
  • Assist with quality assurance initiatives and contribute to continuous process improvement efforts.
  • Protect patient confidentiality and maintain compliance with HIPAA and all applicable privacy regulations.
  • Complete other duties and projects as assigned
  • The ability to adhere to Clarest's Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer

Qualifications:

  • Active Pharmacist license in good standing in the state of practice
  • Minimum of 1 year working in healthcare environment as licensed pharmacist, LTC Pharmacy or related environment experience preferred; formulary/clinical expertise preferred, especially related to Medicare Part D.
  • Strong clinical knowledge and medication therapy review skills
  • Excellent verbal and written communication skills
