Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our partnered nursing homes throughout Saten Island. We specialize in delivering high-quality, compassionate mental health services to older adults in long-term care settings. This position is ideal for a clinician who is passionate about geriatric psychiatry and dedicated to supporting the emotional and psychological wellness of aging populations.
RequirementsPrimary Responsibilities- Conduct detailed psychiatric assessments and follow-up evaluations for elderly residents in nursing home environments.
- Create individualized treatment plans tailored to each patient's mental health concerns and diagnostic profile.
- Provide ongoing psychiatric medication management and make clinical adjustments as needed.
- Work collaboratively with physicians, psychologists, social workers, and facility staff to ensure a coordinated, whole-person care approach.
- Deliver therapeutic interventions, including counseling and supportive psychotherapy.
- Guide patients and families by explaining diagnoses, treatment paths, and strategies for managing mental health conditions.
- Maintain clear, accurate, and compliant documentation within electronic health record systems.
- Participate in team conferences, interdisciplinary meetings, and quality improvement efforts within the organization.
Qualifications- Active New York State Nurse Practitioner license with PMHNP certification.
- Ability to work independently while effectively managing a caseload across multiple facilities.
- Strong diagnostic abilities and familiarity with psychiatric medications and therapeutic approaches.
- Excellent communication and interpersonal skills, with the ability to build trust with patients, families, and care teams.
- Proficiency with EHR systems and basic computer applications.
- Empathetic, patient-focused clinical style.
