Our organization is expanding its behavioral health team and is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our partnered nursing homes throughout Saten Island. We specialize in delivering high-quality, compassionate mental health services to older adults in long-term care settings. This position is ideal for a clinician who is passionate about geriatric psychiatry and dedicated to supporting the emotional and psychological wellness of aging populations.

Requirements
Primary Responsibilities
  • Conduct detailed psychiatric assessments and follow-up evaluations for elderly residents in nursing home environments.
  • Create individualized treatment plans tailored to each patient's mental health concerns and diagnostic profile.
  • Provide ongoing psychiatric medication management and make clinical adjustments as needed.
  • Work collaboratively with physicians, psychologists, social workers, and facility staff to ensure a coordinated, whole-person care approach.
  • Deliver therapeutic interventions, including counseling and supportive psychotherapy.
  • Guide patients and families by explaining diagnoses, treatment paths, and strategies for managing mental health conditions.
  • Maintain clear, accurate, and compliant documentation within electronic health record systems.
  • Participate in team conferences, interdisciplinary meetings, and quality improvement efforts within the organization.
