The Psychiatric Mental Health Nurse Practitioner (PMHNP) Team Lead provides psychiatric care while supporting clinical leadership, care coordination, workflow optimization, and behavioral health program development. This role balances direct patient care with operational and administrative responsibilities, collaborating with interdisciplinary teams and WelbeHealth PACE Physicians to deliver comprehensive, participant-centered care under the direction of the Medical Director.

On the day-to-day, you will:- Provide psychiatric consultation, psychiatric illness evaluation, counseling, and treatment for participants in collaboration with PACE Physicians and Community-Based PACE Physicians and the IDT team to develop a comprehensive care plan for each participant
- Collaborate with physician(s) and IDT in discharge planning from acute or long-term care settings
- In consultation with the PACE Physician and/or Community-Based PACE Physician, refer participants to a psychiatrist as indicated and manage care of participants in the nursing home, making regular visits as dictated by WelbeHealth standards and participant need
- Provide clinical leadership and mentorship to psychiatric nurse practitioners and behavioral health team members
- Serve as the first point of contact for other team members to discuss complex cases and pre-enrollment decisions
- Support onboarding, training, and professional development of behavioral health clinicians and participate in quality improvement initiatives, chart reviews, and clinical audits to promote high standards of care
- Assist with development, implementation, and refinement of clinical workflows, protocols, and best practices and partner with operational and clinical leadership to address program needs, service gaps, and care delivery improvements
Job requirements include:- Masters of science in nursing, with a psychiatric/mental Health emphasis from an accredited college or university
- Certified as a Psychiatric Mental Health Nurse Practitioner
- Active psychiatric mental health nurse practitioner state license without restrictions
- State CDS Certification, NPI and DEA License
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Minimum of four (4) years of experience working with the frail or elderly
- Prior experience serving as a functional lead or subject matter expert preferred
- Minimum of two (2) years working within the PACE model preferred
