Full Job Description
POSITION SUMMARY:

The Nurse Practitioner provides direct patient care commensurate pertaining to his/her training and experience, and in collaboration with CCI physicians. The care provided is appropriate for an out-patient, primary care community health center with a multicultural, multilingual patient population. The Psychiatric Nurse Practitioner (PMHNP) will provide trauma-informed, holistic mental health care to refugees, asylees, and individuals seeking substance use disorder (SUD) treatment. The ideal candidate will have expertise in psychiatric evaluation, psychopharmacology, and integrated behavioral health, with a commitment to improving access and reducing stigma for marginalized communities.

As a member of the health center's integrated care team the Psychiatric Nurse Practitioner will fulfill the following KEY FUNCTIONS AND RESPONSIBILITIES towards the provision of patient-centered, coordinated patient care:
  • Conduct comprehensive psychiatric evaluations and mental status examinations.
  • Diagnose and treat psychiatric disorders, including mood, anxiety, trauma-related, and substance use disorders.
  • Develop individualized treatment plans incorporating evidence-based psychopharmacology, psychotherapy, and community support services.
  • Prescribe and manage psychiatric medications, including MAT (Medication-Assisted Treatment) for opioid and alcohol use disorders when appropriate.
  • Provide crisis intervention and risk assessment for suicidality, self-harm, and psychiatric emergencies
  • Examine, diagnose, and treat patients within the parameters of his/her licensure.
  • Provides telephone, electronic, and/or in-person consultations with providers about specific patient issues. These issues can include diagnostic clarification, next steps, treatment recommendations, assistance with challenging patients, etc.
  • Provides direct consultations for complex patients or patients who are not improving and makes recommendations to the patient's medical provider.
  • Provide treatment to minors with or without accompanying parents.
  • Provides time-limited ongoing care for clients who are high-risk or complex.
  • Completes timely documentation and billing.
  • Complies with meaningful use and PCMH activities
  • Consults on behavioral health systems and processes.
  • Works collaboratively with all providers to ensure coordinated patient-centered activities.
  • Participates in performance improvement activities and other committees and workgroups as needed.
  • Leads by example to provide Center-wide high-quality care that is patient-centered, integrated, and meets CCI's established standards.
  • Adhere to evidence-based guidelines and standards approved by CCI's Board and Performance Improvement Committee.
  • Attend and participate in meetings and/or committees as necessary and/or required by CCI.
  • Participates in CCI programs and partnerships as appropriate.
  • Working on-site is an essential duty for the job.
  • Other duties or responsibilities as assigned by the Medical Director of Behavioral Health.

KNOWLEDGE, SKILLS, AND ABILITIES:
  • Knowledge of state regulations governing advanced practice nursing.
  • Skill in communicating effectively with patients and their families.
  • Skill in establishing and maintaining effective working relationships with other employees, patients, and the public.
  • Computer skills required. Knowledge/experience with Electronic Medical Records (EMR) is a plus.
  • The ability to speak and write English fluently. Bilingual (English and Spanish) preferred.

MINIMUM QUALIFICATIONS:
  • Nurse Practitioner certification from an accredited program.
  • Specialty Certification in Psychiatry required.
  • Active license issued by the State of Maryland Board of Nursing.
  • Active Maryland CDS license. DEA licensure. Current BLS certification.
  • Ability to relate to culturally diverse patients and community.
  • Flexibility to work at different CCI locations.
