Program Manager, Care Operations-

Role Overview

We're looking for a Program Manager to execute and scale Chamber's AI-enabled cardiovascular care programs. Reporting to the Director of Program Operations & Infrastructure, this role works across Clinical, Product, and Care Delivery Operations translating protocols into workflows, driving cross-functional execution, and creating structure where ownership is still being defined.

This is a role for someone who moves work forward without waiting for perfect conditions, brings clarity to ambiguous situations, and follows through. We have active contracts and attributed populations today and the programs serving them need to run well and keep improving.

Key Responsibilities

Program Execution & Cross-Functional Coordination
  • Own end-to-end execution of care programs from workflow documentation to go-live tracking to post-launch iteration.
  • Coordinate across Clinical, Product, Engineering, and Care Delivery Operations to keep programs on track, identifying blockers and closing gaps.
  • Manage program timelines, deliverables, and status tracking for active care contracts and attributed populations.
  • Serve as a consistent point of contact for cross-functional stakeholders, keeping everyone aligned on priorities and progress.

Workflow Design & Documentation
  • Translate clinical protocols and product capabilities into operational workflows the Care Delivery team can execute consistently.
  • Build and maintain SOPs, playbooks, and training materials that allow programs to run reliably across care team members and markets.
  • Identify workflow gaps, unclear handoffs, and missing infrastructure and take ownership of closing them.
  • Partner with the Associate Director, Care Management on workflow and SOP documentation and process iteration, ensuring documentation reflects how care teams actually work.

AI Tooling & Workflow Integration
  • Partner with Product and Engineering to integrate AI tools into care workflows, understanding where automation handles scale and where human intervention is needed.
  • Support configuration, testing, and rollout of new tools or workflow changes.
  • Track operational performance and surface opportunities to reduce manual burden through process improvements or tooling changes.

What You'll Achieve in Your First 90 Days
  • Review and document the workflows, staffing, and tooling for each active care program, and produce a written summary of gaps and immediate priorities.
  • Build or improve core operational infrastructure including SOPs, workflow docs, and escalation paths, for clinical programs
  • Build working relationships across Clinical, Product, and Care Delivery and become a reliable point of coordination.
  • Deliver a status update on program execution: what's working, what isn't, and what the plan is to address it.

Requirements
  • 4-8 years of experience in healthcare operations, program management, or an adjacent field. Value-based care, population health, or managed care experience preferred.
  • Demonstrated ability to create structure in ambiguous environments - you don't wait for ownership to be assigned before driving work forward.
  • Strong process documentation skills - your SOPs and playbooks are ones teams actually use.
  • Experience working with care management platforms, AI-enabled tools, or operational technology in healthcare.
  • Analytical and data-driven - you use metrics to track progress and make decisions.
  • Clear, direct communicator who keeps stakeholders informed and aligned.
  • Passionate about improving outcomes for cardiology patients and providers.
