We’re looking for a Product Manager, Total Cost of Care to add to our team as we expand on our technology offerings.
Position Summary
In this role, you will define and deliver technology-enabled solutions that improve outcomes for Integrated Kidney Care patients while reducing avoidable healthcare utilization. This role will partner with other teams across the organization to ensure IKC field teammates have the right tools to identify high-risk patients, prioritize outreach, coordinate care across roles, and execute interventions that help patients spend more days at home and fewer days in the hospital.
Key Responsibilities
- Drive product strategy and roadmap execution in the Total Cost of Care space, including tools and workflows that support hospitalization reduction and chronic care management.
- Translate clinical and operational goals into product requirements through a deep understanding of IKC workflows, pain points, data needs, and patient outcome objectives.
- Prioritize work against measurable outcomes, including reduced avoidable admissions, improved timely outreach, increased care team efficiency, higher-quality patient conversations, and better use of clinical and non-clinical resources.
- Ensure products are supported by accurate, timely, and actionable data from internal and external sources.
- Lead discovery and solution design through user interviews, workflow mapping, data analysis, pilot feedback, and cross-functional working sessions.
- Manage product delivery by writing clear requirements, refining backlog items, supporting sprint planning, making tradeoff decisions, validating completed work, and driving adoption after release.
Required Qualifications
- Bachelor’s degree in business, healthcare administration, product management, or related field; equivalent experience will be considered.
- 3+ years of experience in product management, healthcare technology, clinical operations, analytics, or related roles.
- Ability to facilitate communication across clinical, operational, technical, analytics, and executive stakeholders.
- Comfort using data to frame problems, evaluate tradeoffs, monitor product performance, and demonstrate outcomes.
- Strong judgment in balancing patient impact, teammate experience, delivery considerations, and business value.
- Agile experience: user-centered discovery, backlog prioritization, user stories and acceptance criteria, scrum team partnership.
Preferred Qualifications
- Experience in value-based care, population health, kidney care, chronic disease management, and/or care management.
- Experience with EHR or care management platforms such as Epic, Salesforce, or similar systems.

This position will be open for a minimum of three days.
The Salary Range for the role is $78,000.00 - $119,000.00 per year.
