CenterWell / Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Jupiter, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach. As a Primary Care Physician, you will report to our Associate Medical Director. This role will be onsite and in-person at the Conviva Military Trail Center in Jupiter Florida. A day in the life of a CenterWell physician 3 Monday-Friday 8:00am-5:00pm3 Join an experienced team with an established patient panel3 See 14-17 patients per day. Opportunity to spend more time with patients3 Longer authorized appointments (45 minutes)3 Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care Required Qualifications 3 Doctor of Medicine or Osteopathic Medicine3 Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine3 Valid, unrestricted license in the state of desired employment (or willing to obtain)3 Current, unrestricted DEA license3 If selected for this role, you will be required to be screened for TB Preferred Qualifications: 3 Minimum of two+ years of experience3 1+ years of experience managing Medicare Advantage panel of patients3 Knowledge of Medicare guidelines and coverage.3 Knowledge of HEDIS quality indicators.3 Fluent in Spanish and English Use your skills to make an impact 3 Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $255,000 - $298,500 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
