What You'll Be Doing
The Advanced Practice Provider (NP or PA) will serve as the dedicated on-site clinician at an upscale subacute rehabilitation and transitional care facility in Northeast Atlanta. Unlike our traditional APP model, this clinician is in the building for a full 8:00 a.m.-6:00 p.m. shift, four days per week, weekdays only, with no night, weekend, or call coverage. The facility is an all-private-suite building known across metro Atlanta for pairing strong clinical outcomes with a hospitality-driven experience - chef-prepared dining, concierge services, and an expectation that every patient and family is treated like a guest. We are looking for a clinician who delivers excellent medicine and an equally excellent experience. You will collaborate with facility leadership, therapy partners, physicians, and hospitality staff to ensure timely, effective, and coordinated care.
Key Responsibilities
- Provide direct medical care to a short-stay, post-acute rehabilitation population - including orthopedic, cardiac, pulmonary, stroke, and neurologic recovery patients - including assessment, diagnosis, treatment, and follow-up.
- Serve as the consistent, visible clinical presence in the building throughout the 8:00 a.m.-6:00 p.m. shift, available in real time to patients, families, and facility staff.
- Round proactively: meet every new admission, set expectations for the stay, and check in with patients and families before they have to ask.
- Manage acute and chronic conditions on-site, avoiding unnecessary transfers and hospitalizations whenever it is safe and appropriate to do so.
- Coordinate with facility leadership, therapy teams, specialists, and families to develop and update individualized care plans.
- Support transitions of care, including post-hospital evaluations, medication reconciliation, and patient/family education through discharge home.
- Partner with the interdisciplinary team - nursing, therapy, social work, and concierge/hospitality staff - to deliver a seamless, high-touch patient experience.
- Communicate promptly and professionally with referring surgeons, hospitalists, and primary care physicians who entrust their patients to the building.
- Maintain thorough, timely, and compliant documentation of patient encounters and care plans.
- Participate in quality initiatives, patient satisfaction efforts, and process improvements that enhance outcomes and the guest experience.
Skills & Abilities
- Exceptional customer service orientation - you naturally treat patients and their families as guests as well as patients, and you see service recovery as part of the job.
- Strong clinical knowledge of adult and geriatric medicine, with expertise in post-surgical, post-acute, and chronic disease management.
- Excellent communication and collaboration skills, with the poise to manage high expectations from discerning patients, families, and referring physicians.
- Ability to manage a patient panel efficiently while maintaining a high standard of care and an unhurried bedside manner.
- Comfort being the face of the medical team in a building where hospitality standards are as visible as clinical ones.
- Strong organizational and documentation skills.
- Commitment to patient-centered, evidence-based practice.
Qualifications
- Current Nurse Practitioner or Physician Assistant license in the state of Georgia.
- For NPs: certification in Internal Medicine, Geriatrics, or Family Practice required. For PAs: current NCCPA certification required.
- Must have an unrestricted DEA license
- Minimum 1-2 years of NP or PA experience preferred; experience in subacute rehab, post-acute, SNF, or geriatrics strongly preferred.
- Ability to work independently in a facility setting with physician support.
- Familiarity with EHR systems and clinical documentation best practices.
- Experience in a concierge, boutique, or service-forward care environment a plus.
