Qualifications

• Master's degree as a Nurse Practitioner (NP) or Physician Assistant (PA) with active Texas licensure.
• Experience in managing chronic and acute medical conditions, particularly in home settings.
• Strong communication skills for effectively building trust with patients and families.
• Ability to address social determinants of health impacting patient care.
• Proven ability to thrive in a teamwork-oriented, interdisciplinary environment.

Responsibilities

• Serve as the primary care provider for a specific patient panel in home and community settings.
• Diagnose and treat medical conditions with clinical support from a multidisciplinary team.
• Assess and address social determinants affecting patient health and well-being.
• Conduct specialized in-home visits for wound care and transitional care management.
• Complete comprehensive evaluations to enhance patient health and safety.
• Leverage integrated data systems for tracking patient outcomes and coordination of care.
• Collaborate with internal and external teams for seamless patient transitions and continuity of care.
