Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care Advanced Practice Provider (APP) for our clinic in Pompano East, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. As a Primary Care Physician, you will report to our Associate Medical Director.
Key Responsibilities- Provide comprehensive primary care services to adult and geriatric patients, including assessments, diagnosis, treatment, and ongoing care management
- Manage chronic conditions with an emphasis on preventive care and wellness
- Perform physical exams, order and interpret diagnostic tests, and develop individualized treatment plans
- Collaborate closely with physicians, care coordinators, behavioral health specialists, and clinical support staff
- Focus on quality-driven outcomes, including HEDIS measures, CMS Star initiatives, and population health goals
- Educate patients and caregivers on disease management, medications, and healthy lifestyle choices
- Maintain accurate and timely documentation in the electronic medical record (EMR)
- Participate in care team meetings and quality improvement initiatives
A day in the life of a Conviva APP- Monday-Friday 8:00am-5:00pm

Qualifications- Active, unrestricted Nurse Practitioner license in the state of practice
- National certification (AANP or ANCC)
- DEA license (or ability to obtain)
- Minimum of two+ years of experience
- 1+ years of experience managing Medicare Advantage panel of patients
- Knowledge of Medicare guidelines and coverage.
- Knowledge of HEDIS quality indicators.
- Fluent in Creole and English

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$123,800 - $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
