We are currently looking for a Full Time Primary Care Advanced Practice Provider to join our Division of Ambulatory and Public Health team located at the Purcell Health Clinic working .
Summary:
Provides a full range of care, evaluate and treat acute and minor illnesses or injuries as well as a variety of primary care of episodic complaints.
Responsibilities:
* Maintains confidentiality, security, and integrity of data and information in accordance with HIPAA regulations
* Performs assessments on patients experiencing urgent, emergent, or non-emergent conditions
- Assesses physical needs of all ages of patients
- Assesses bio-psychosocial needs of all ages of patients
- Assesses health education needs of all ages of patients
* Facilitates consults, referrals, and form completion as requested and approved by the medical staff
* Evaluate and treat all patients for acute minor illnesses or injuries and provide care for primary care of episodic complaints
* Provides preventative diagnostic and therapeutic care and services for patients of all ages
* Participates in performance improvement activities to measure, assess and improve the quality of assigned work area
* Ensures continuity of care and provides timely requests for schedule changes, vacation, sick leave and continuing medical education leave to leadership
* Provides first class customer service to both internal and external customers and responds to inquiries, compliments, and/or complaints with professionalism, courtesy, and efficiency
* Completes all training and development requirements and participates in continuing education opportunities
* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types

- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
