Full Job Description
About the Role:

We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible.

We are currently accepting applications for a Modified FTE (32 hrs/week) Obesity Medicine Physician opportunity, who are licensed in MIssissippi (MS), OR hold an active IMLC (Interstate Medical Licensure Compact) designation, or possess an official Letter of Qualification (LOQ).

This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.

This is a Modified Full Time position based in a 100% remote (must be based in the US), telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties.

  • At this time, this role does not collaborate with APPs. In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
  • Provide treatment planning and medical management support for complex cases or escalated concerns.
  • Prescribe non-controlled obesity medications for patients under direct care
  • Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
  • Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities.
  • Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
  • Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.

  • This position requires multi-state practice capability. Applicants must either already hold a valid Letter of Qualification (LOQ) through the Interstate Medical Licensure Compact (IMLC) or meet all prerequisites to rapidly secure one upon hire
  • Current or previous credentialing with insurance payers is desired, but not required
